Student Accident Insurance

Illinois law mandates that an accident insurance policy be offered to students at the beginning of the school year at a nominal premium. H-F High School has purchased basic accident insurance coverage for all students attending our school. This insurance covers accidents that may occur on school premises. If you are interested in purchasing extended coverage for your student, please click the links below:

Please note: It is the responsibility of the student to report all injuries, immediately, to the teacher, sponsor or coach. An accident form will be filled out and forwarded to one of the nurse's office, or the athletic office for sports-related injuries. Claim forms will be sent out to the parent/guardian, based on the filled-out accident form. This claim is to be filed by the student's parent or guardian. The school does not file insurance claims.

 

Students with disabilities who do not qualify for an individualized education program, as required by the federal Individuals with Disabilities Education Act, may qualify for services under Section 504 of the federal Rehabilitation Act of 1973 if the child (i) has a physical or mental impairment that substantially limits one or more major life activities, (ii) has a record of a physical or mental impairment, or (iii) is regarded as having a physical or mental impairment.
