COPY OF BIRTH CERTIFICATE
PARENT/GUARDIAN DRIVER'S LICENSE OR STATE ID
REQUIRED PROOF-OF-RESIDENCY DOCUMENTS
One of the following documents is required:
- Current mortgage statement or closing documents from home purchased within the past 2 months or current homeowner’s insurance policy (in lieu of mortgage statement).
- Signed and dated current lease.
One of the following documents is required:
- Most recent ComEd bill
- Most recent Nicor bill
One of the following documents is required:
- In-district current vehicle registration
- Current auto insurance policy
- Most recent cable or internet bill
- Most recent cell phone bill
- Most recent water bill
- Current homeowner/renter’s insurance policy
- Bank statement
- Check stub

*Students who do not participate in February’s Incoming Freshman Registration will complete their enrollment and registration after they graduate from junior high school. Registration for the 2022-23 school year will begin after June 20, 2022.
