(Fees:  Pick-Up/Regular Mail is $3/copy, Certified Mail is $7/copy

Mail the completed form with a check to:

                          H-F High School Transcript Request
                          999 Kedzie Ave.
                          Flossmoor, IL 60422
                          attention: Registrar’s Office

or

You may complete the form and return it in person to the Guidance Office and pick up your transcript on Thursdays between 9 a.m.-3 p.m.
