Proving residency in District 233 for new students is done by appointment only. Please call 708-335-5594 to make an appointment. New enrollment to H-F is a three-day process – (1) proof of residency; (2) placement test; (3) counselor’s appointment.

Proof of Residency Checklist
[  ] Copy of monthly mortgage statement or recent closing papers, or current original lease with name
      and telephone number of the landlord.
[  ] Required:  Utility Bill - NiCor or ComEd
[  ] Supply one of the following:
      □       Water hook-up with Village verification
      □       Current bank statement with in-district address
      □       In-district vehicle registration
      □       Check stub with name and in-district address
[  ]  Supply one of the following: 
       Current Auto Insurance Policy or current Homeowner Insurance
[  ] Illinois Driver’s License with in-district address
[  ] Student’s Birth Certificate
[  ] Transcript from previous high school or elementary school diploma with last report card
[  ] Student  State Transfer Form from previous high school along with a copy of most recent physical,
        and complete discipline records
[  ] Divorce Decree (first page, last page and the section showing custody) if applicable
[  ] Court Ordered Guardianship Papers, if applicable
[  ] If an incoming Freshman, a recent physical with updated immunizations

If any of the above information is missing it will result in a delay in the enrollment process.
